    You can conveniently complete it here in advance or, alternatively, at your first visit to SGK.

    Our tip: it is best to complete the form online straight away. This saves you time on arrival and allows us to prepare your visit as well as possible.

    You measure blood pressure and pulse at home, first lying down, then standing for ten minutes, and enter the values. Instructions and immediate evaluation are included. You need a blood pressure monitor and a second person.
