By signing the personal details form, I confirm that I agree to the processing of my data, to the doctor's access to the data and to the disclosure of the data to third parties as described in the patient information on the following page.
I am aware of the possible risks of exchanging particularly sensitive personal data (possible access by unauthorised third parties via insecure channels of communication) and of my rights, and I consent to contact between my doctor and me as a patient via the contact details given above. The practice passes on patient information exclusively via secure channels. I agree that administrative matters, such as changes to appointments, may be dealt with by unencrypted e-mail.
The Federal Health Insurance Act (KVG) provides that patients receive a copy of the doctor's invoice.
Appointments not cancelled at least 24 hours in advance may be charged to you as a private service. Please let us know in good time.
Patient information on the handling of personal data
Below we inform you of the purposes for which the above-mentioned medical practice (hereinafter "the practice") collects, stores or passes on your personal data. We also inform you of the rights you can exercise under data protection law.
Responsibilities
The practice is responsible for processing your personal data, and in particular your health data. If you have questions about data protection or wish to exercise your data protection rights, please contact the practice staff or your doctor directly.
Collection and purpose of data processing
Your data is processed (collected, stored, used and retained) on the basis of the treatment contract and legal requirements, in order to fulfil the purpose of treatment and the associated obligations. Data is collected on the one hand by the treating doctor in the course of your treatment. On the other hand, we also receive data from other doctors and health professionals who have treated or are treating you, if you have given your consent. Your medical record only contains data related to your medical treatment.
The medical record includes the personal details given on the patient form, such as personal, contact and insurance details, as well as, among other things, the information given to you during treatment and the health data collected, such as medical histories, diagnoses, treatment proposals and findings.
Retention period
Your medical record is kept for 20 years after your last treatment. After that, it is kept longer with your express consent, or securely deleted or destroyed.
Disclosure of data
We only pass on your personal data, and in particular your medical data, to external third parties if this is permitted or required by law or if you have consented to its disclosure in the course of your treatment.
- Data is sent to your health insurer, or to accident or disability insurance, for the purpose of billing the services provided to you. The type of data transmitted is based on the legal requirements.
- Data is passed on to cantonal and national authorities (e.g. the cantonal medical service, health departments, etc.) on the basis of statutory reporting obligations.
- Optional: the necessary patient and invoice data is passed on to a debt collection agency for the purpose of collecting overdue amounts.
In individual cases, depending on your treatment and your consent, data is sent to other authorised recipients (e.g. laboratories, other doctors).
Withdrawing your consent
If you have given your express consent to data processing, you may withdraw it in full or in part at any time. Withdrawal or a request to change your consent must be made in writing. As soon as we receive your written withdrawal, processing will stop, unless it can be based on a legal ground other than consent. Withdrawal does not affect the lawfulness of processing carried out before it.
Information, access and copies
You have the right to obtain information about your personal data at any time. You can inspect your medical record or request a copy. There may be a charge for the copy; any costs, which depend on the work involved, will be communicated to you in advance.
Right to data portability
You have the right to have data that we process automatically or digitally handed over to you or to a third party in a common, machine-readable format. This applies in particular to the transfer of medical data to a health professional of your choice. If you request the direct transfer of the data to another controller, this will only be done where technically feasible.
Correcting your data
If you find or believe that your data is incorrect or incomplete, you can request a correction. If neither the correctness nor the incompleteness of your data can be established, you have the right to have a note of dispute added.
